Hearing about djmarek's problem with his UP visa inspired me to start organizing files and make sure I could pull enough proof of cohabitation documentation.  I'd like feedback on whether I have everything covered.

If all goes well, we'll be applying for my gf's UP visa at the end of June.  We've been living together continuously since Aug 2002, but I'm starting the documentation at May 2003.

I have things organized as file folders (some of them are empty at the moment, but I'll report what will go there).

GF's identity documents
- US passport

Proof of residence, May 2003-April 2004 (all documents from May 2003 unless otherwise specified)
- for Me
    * bank statement
    * credit card annual statements (2002 and 2003)
    * car insurance bill
    * payroll advice of deposit
    * voter registration card (dated 06/18/2003)
- for GF
    * bank statement
    * payroll advice of deposit
    * 2003 W-2 (from Jan 2004, usual W-2 time)
    * voter registration card (dated 06/18/2003)

Proof of residence, May 2004-Jun 2005
- for Me (all from May 2004 unless otherwise specified)
    * bank statement
    * credit card annual statement (2004)
    * car insurance bill
    * payment stub
- for GF
    * bank statement
    * payroll advice of deposit
    * 2004 W-2 (from Jan 2005, usual W-2 time)
- for Me/GF, jointly
    * bank statement (June 2004)

Financial Info
- Recent bank statements (3 most recent)
- Letter of UK employment for me, showing salary
- Employability info for GF
    * CV
    * recent paystubs showing US salary

UK Residence
- lease, with both/only GF and me listed

Sponsor Info
- my UK passport
- letter of sponsorship (is this needed if I am going with her?)

Anything else I need?  I also had phone and credit card bills, but because the stub was removed and sent with payment, there's no address info on them any more!

life insurance policies?  i have a private one outside of my company... and it's really cheap ($15/mo for $100,000)
had a bit of a wobble.

My gf is the beneficiary on my retirement plan.  But I don't know if I have a copy of anything showing that.  I think I could perhaps print the screen from my account online that shows it.  Do you think that would be okay?

What else could I present to demonstrate our relationship?  Letters from friends/family?

ETA:  I found an official document from my retirement fund showing my GF as my primary beneficiary (and her relationship as "partner") and added that to my files.

This is pretty much what BF and I had.  I won't add anything for those years you mentioned.  You may consider adding the 2002 year if you have the proof, but really it won't be necessary.  I wouldn't bother with letters from friends and family. I didn't have anything like that and we sailed through.
