Has anyone successfully managed to get Health Insurance in the UK as an American on a spouse visa?

It would seem that a requirement for health insurance is actual UK citizenship.

Can anyone shed some light on this?

Are your looking for travel insurance or private cover while in the UK?

Try searching under the wording Comprehensive Sickness Insurance as the insurers who sell those cover non-Brits who live in the UK. CSIs are private health cover required by EU citizens and their families who reside in the UK, but who are not allowed to use the NHS for free.
