Spouse or unmarried partner?

Hello,

I'm an American married to a dual-US/UK-citizen and we are currently living in Israel and planning to settle in the UK.

We got married in London in 2010 with a (Jewish) religious ceremony (we couldn't get legally married as we were both under 21) and in Israel we have received a court ruling that our marriage in London is to be recognized by the Israeli government. (The US Embassy in Israel would not accept it as a marriage certificate for our child's passport, but the US embassy in London did)

My question is, do I apply as a spouse or unmarried partner?

Also, what evidence do I need for this segment of the application? (We have three children with birth certificates stating both of our names)
