Get a new one if it leaks. They're cheap enough. They're usually out as Christmas gifts this time of year too, with cute little covers. 
I had one without a cover and I used to wrap it in a towel. One night, I kicked the towel off and I didn't realise I had.  I ended up with a huge burn on my leg.  So I I promptly got one with a cover.
I've never gotten food on my underpants!

I wholeheartedly recommend Fashy hot water bottles.  :)
 
They are made of a different kind of material, not rubber so don't have that nasty smell, also the hole for filling is larger so there is less chance of burning yourself with the splash back.
They're a couple of pounds more expensive than the others but they last much longer too.

They come in different sizes and colours and with/without covers, single or double ribbed or plain.  I bought my husband the clear one with floating fish in it last Christmas!  ;D
