Skilled Worker Visa

In the USA if an organization wants to hire a skilled worker from abroad, they pay for any needed visas or government paperwork.  In the UK does this apply, or does the future employee pay for the skilled worker visa?

What is the difference between being on the shortage occupation list and the skilled worker immigration salary list? Is the application fee lower if the job is on the immigration salary list?

Am I correct in understanding that the skilled worker visa is good for five years (renewable) and that the visa holder could apply for settlement after the five year mark?
