Lol, the thought crossed my mind but we all know what the saying is about making assumptions.
Also for the family name, I have been doing it like this: Jane Holly
Jones (Smith)
Where Jane and Holly are given names, Jones is the married name with a double underline beneath it, and Smith is the maiden name in parens. It's reversed if there's a patronymic or the maiden name is double barreled.
To be honest, it doesn't matter much in the larger picture, but it's good to stay consistent with whatever convention you adopt.
OH! And yes, full name on 2.2 is all of your given names. Whereas full name on 1.1 is - in fact - your full name.

The whole form is badly in need of an overhaul.
