Anyone change name by deed poll? - US passport question

I am thinking of combining my and my husband's surnames (either hyphenating or changing my maiden name to my middle name.)

I know that I need a deed poll to do this, and I emailed the US Embassy in London asking if it was acceptable to submit a copy of the deed poll to get my name amended on my US Passport.

They emailed me back with a link to a page on the US Embassy website where it tells you to submit your marriage certificate to show proof that you changed your name upon marriage. Not very helpful at all.

So, anybody who changed your name by deed poll, how did you get your US passport amended?

Actually, deed poll isn't accepted by the American Embassy for a name change on your passport.  You need to do a Statutory Declaration.  If you're interested, once you're at that point, I can prepare the forms for you (free of charge of course!) and you can take them to a solicitor or a court and get them signed.  A good friend drew them up for me and it cost me £8 each to get my two sons' done.  I've drawn up the papers for another ex-pat and her daughter as well and they had no problems.

If you just want to take on your husbands surname all you have to do is fill out the DS-19 and send in your marraige cert.

That's true, too.  I was actually thinking about my answer to this last night (lost the fight with the 16-year-old for computer time!).

If you want to take your husband's last name and hyphenate it with yours, I think all you have to do is show the marriage certificate and fill out the form the way you want it to appear.  Personally, I dropped my maiden name altogether, so my name is First Middle Married.  And all I did was show my marriage certificate and tell them that's how I wanted it.

I had to do stat decs for my sons because my husband hasn't actually adopted them legally, but since they had my maiden name, I (and they) wanted them to have my married name.

Anyway, whatever you decide to do, if you need a stat dec to do it, the offer is there!
