Another FLR question

I'm in the middle of filling out my FLR(M) application and I'm stumped on some questions.

1-4 Home Office Referance- What do I put?

 2-1 Full name question- Do I put my maiden name thats on my passport or my married name?

3-2 Do you or your spouse have any children under 18 from your marriage or relationship who are dependant on you
or your spouse - Do I put down my daughter,shes a UK citizen or is this for children who need a visa?

and finally do I have to send in my divorce certificate from my previous marriage?

There's no fixed rule about this.  I advise that you put the name you want to use in the UK hereafter, and use your marriage certificate to reconcile any differences...
