Custom forms for shipping via USPS

I have read the instructions on these forms several times, but I just can't figure out what I do. The post office gave me 1 envelope pouch, 1 Customs declaration and dispatch note and 1 customs declaration. I know that many of you have shipped your stuff this way. I was wondering  if any of you might know the answer to these questions.....

1. Do I really have to list every item in the box on the form?
2. Do I have to enter the value for each item or can I just do one lump sum?
3. Does 1 copy of the form go in the box and all of the rest in the envelope pouch?

Thanks for your help! I have read the instructions over and over and I am sure I am doing this wrong. I think I am just really stressed out.

I personally have never listed every item---I have always kept my description quite general ie: household items, decorations, clothing
I tend to put a couple general descriptions and then a couple of prices.
I am sure this is not the way you are officially supposed to do it--but I have never had any problems.

the company that is shipping my boxes told me to mark contents like ... kitchen goods, clothes, books. etc
I didnt put value's, they told me to put "used household goods"
I dont know how different USPS is though.   :(

something to help feel less stressed..... stand up, stretch, and nice deep breaths!  (sounds cheesy but I find myself doing it often since im always feeling overwhelmed lately!!)   :P

We just had 24 boxes arrive a few weeks ago via USPS. Nothing was priced or insured and we gave very brief descriptions as to the contents. We only filled out one customs form and let the post office deal with the pouch and the extra copies. They'll have to stamp each page of the form so you won't want to tear it apart.
Also one postal worker told us that none of the boxes could have writing on them (this was after another post office let us mail numerous boxes with writing on them), so you may want to have a sharpie close by just in case.
