Now... what I would like to know is if anyone has any useful example of the kind of notarized letter that he should sign in order for us to get a visa for Jesse?
Also, If anyone could clarify exactly what we need for us to get the visa's without any trouble that would be great... There is a likelyhood that the visas will be applied for in person in New York.
Any other information you may be able to give would be wonderful. I do require this information asap... as the situation is moving quickly and we must keep up.

I simply typed a letter, proper letter format with my daughter’s dads name, address, etc., like this:
To whom it may concern:
This notarized letter is to inform you of my approval and permission to allow my daughter, (name), passport number xyz, to accompany her mother, (my name and passport number) in the application of any visa(s) necessary so that they may travel abroad and reside in the UK.
His signature
Then the notary attached their paper to that letter. I had absolutely no problems with that.
Organized neatly in a binder, I had a a cover letter outlining the document contents, letter of how we met, bank statements, phone bills, emails, pictures, notarized letter from my daughter's dad, divorce papers, proof of Kevin's ownership of property, my work stuff like CV, etc., Kevin's work contracts, and other misc. stuff.
