quick question about the tick and worm treatment

I'm taking our cat in this morning for his tick and worm business. I have the 3rd Country form with its tick and worm sections, but I seem to remember downloading another form specifically for the tick and worm treatment, also to be filled out by my vet. I can't find this form anywhere. Am I imagining this or is there a separate form? If so, how can I get another copy?

I'm useless these days and can't solve a simple problem to save my life.  :P

We were supposed to be a the vet's 10 minutes ago but our cat went out this morning to do his usual business and hasn't returned!

No extra form, just the third country vet cert.  Make absolutely sure that the vet writes the date in long form and especially the TIME in 24 hour clock so as to avoid confusion.  The AHR says that the most slip ups happen with the vet not putting in the date and time correctly.

Thanks, Mrs. Pink. The cat came back and we've been to the vet for his tick and worm. No fecal samples required, right? Does he need to stay indoors for the duration? Not a bad idea, considering this morning's ordeal.
