DS-2001 Checklist Question

This is the checklist I mail to the Embassy to let them know we're ready for the interview. I know I'm not supposed to send original documents with it, since we bring those to the interview. I also know that at the interview I'm supposed to make photocopies of the originals and have them paperclipped to the actual originals.

QUESTION: When we mail the checklist, do they want photocopies of our evidence, or do we JUST send the checklist with little tick marks beside what we'll bring?

They will ask for everything at interview.
Make certain you have an extra passport photo for the doctors medical exam (they dont list that you need one)

They were very picky about the photos.  We had it done by a photographer with the measurements they listed.. and STILL they didnt quite meet what they wanted and sent hubby out to Oxford Street to get another set.  But it was no problem and took about 4 minutes.
