Medical Records

Not sure where this fits, but its healthcare related,so popped it here.

I got a letter today, from a hospital I use to go to for many years for surgery and etc. Its a medical records release form for me to sign so the hospital here can have it.
 
I'd like a copy of it myself so I can see my medical history (long story about things not being disclosed to me when I was a child) Does anyone know if hospitals can release my medical records to me for my own use or do they just release it to hospitals?

Figured I'd ask here instead of charging up my phone bill making a phone call to Baltimore!

i needed my medical records from a hospital in boston a few years ago.  i had to sign a release form... but photocopies were given to me for my own use.  no problems.
If you harbour bitterness, happiness will dock elsewhere.

I think you have a right to have a copy of your medical records...
