APHIS FORM 7001

Hello,

I have a question regarding the APHIS Form 7001. If I understand it correctly, this form is needed when bringing pets to the UK on PETScheme. Where do I get that form? I spoke to my vet and she has no idea what I am talking about. I also phoned local USDA office and British Airways' Pet Hotline (our flight is booked with them), and both say that I do not need the APHIS Form 7001. According to them, all I need is the EC 998/2003 certificate (including supporting documents) and health certificate. I am confused now. Is this new rule or am I getting wrong information from both, USDA and British Airways?

I got my APHIS Form 7001 from my Vet.  Apparently, it's a form that only some airlines require...according to a recent thread on here http://talk.uk-yankee.com/index.php?topic=32288.0 anyway.  You should be able to trust your airline, I would think they would have given you the correct information.
