I was happy with UPAKWESHIP.  -good price and good service when I called her.  She got me all the necessary forms.  The company on the UK side did try to send me some paperwork and implied I had a fee, but I emailed the US company, and she had it all sorted within hours.  It did take longer than expected as mine also got held up at port, but the service was door to door (they even carried it up to my second-third floor flat), and I only had one item broken (due to my packing).

Did you order the packing material from the shipping company?

No, that would have been very difficult as they were in another part of the country.  I just bought boxes from Uhaul.
