Maternity Allowance (MA1)...anyone with experience with this?

I'm a supply teacher and have received a denial for SMP from my employer as my 'earnings were too low' to qualify. In return, I am having to file for Maternity Allowance, which I understand to be an exception to the 'no public funds' clause on my FLR(m) status.

I'm doing the online PDF (filling it in on screen and saving to print off) and I was wondering if anyone has had any experience with these forms or claiming Maternity Allowance. It is asking questions about time spent outside the country in the 4 years leading up to my claim and since I have only lived in the UK now for 14 months, I naturally resided previously in my home country (US).

I just want to make sure I am filling out this form correctly but it is assuming I was only "visiting" a country if I was living outside of the UK.

Anyway...guidance would be MUCH appreciated if anyone has had past experiences with this.

26 Oct 10 - ILR app (posted Special Delivery)
27 Oct 10 - online tracking confirmed delivery
30 Oct 10 - Confirmation via post
15 Nov 10 - ILR granted/documents returned!!!

05 Nov 13 - 1st daughter born :)

I'll go dig out my copies, as I was definitely here less than 4 years when I filled mine out.
