Question about form DS-2029 (reporting birth etc)

I have an appointment at the US Embassy and I am filling out for DS-2029, and I am confused with this one...
Item 12 - Physical Presence: This is the actual time when the parent was physically present in the United States. This means that any travel outside the United States, including vacation, should be excluded. Maintaining a residence in the U.S. does not constitute physical presence. Please use the space on the form to list all periods in which you were physically within the borders of the United States to the best of your memory. Start with your birth, if you were born in the United States, or your first entry to the United States, if you were born abroad, and account for all periods up to the present. If possible, please use the space on the form rather than attaching a separate page with this information.

How do you fill this in if you have been travelling betwen the US and the UK for the last 15 years? I seem to remember that when I went to the Embassy last time, they simply asked me how many days I had been out of the UK. Do you know what I should do?

Also, if I use my parents address in the states on my passport etc, does that count as keeping a residence in the US?

Since it says "Maintaining a residence in the U.S. does not constitute physical presence."  I'd say parents home doesn't count.

If I recall rightly I put something like May 1972 - April 2003
April 2003 - October 2003
October 2003 - Jan 2004 etc etc. (for all my visits)
 ending with May 2005 to 3 September 2005 (when I moved to UK on fiancee visa)

Can't recall if I included the week in Dec 2007 we went to MA for a visit, probably did.
