Looking at the Annex II (pets go to vet tomorrow) and I am confused by 1.1 Consignor and 1.5 Consignee. I am the responsible party in the US and in the EU. Can I fill in my name for both? Also I don't have an address or phone number in the EU. Can I put my work information? Thanks!
