I wonder if I have a different one than my original. I lived in an area with a high number of students from other countries and didn't reply to a mailing they sent out asking me to contact them to verify I was still in the country, since I was an expat. I then went in for a maternity visit only to find I was no longer in the records. It seems they had to periodically clean up the records so they didn't fill up the books with non-existent patients. They had no trouble registering me again though.
