Payment Details flrm, section 12.1 help and Documents requied

Hi

I will be making my application in person at premium service centre. I have paid an online payment.

Do I need to fill in the Payment details  give my card details on the form.

Do I need to answer 12.1
 
12.1 Is your current passport, travel document or national identity card enclosed there no box

Do I need to provide a passport for a child who is named the application , but is not applying. she is a British national.

You do not need to fill in the payment information.  Just bring the confirmation of payment with you.

For 12.1, I'd just write Yes next to it.

You need your child's birth certificate (passport not required).
