First off, are you aware that there is a new FLR(M) form, published a couple of days ago - version 04/2016?
No I was not aware, however I signed up last month when the cost had not changed yet...Therefore, would I still take the old one?
1. No, you don't fill out the payment page, just take your proof of payment with you
Okay thanks
2. You need to answer yes to 6.17 - because you have to show proof of the address where you will live together for the accommodation requirement. What address are you providing for your accommodation requirement evidence and your permission to live in the same property when you have finished your studies? That's the address you need to use.

My accommodation is on campus housing and that is the address I am using for everything. And how can we get permission for something when we plan on moving to whole new house when my studies end.... in another 6 months... we do not have proof of a signed lease for that.
3. Just fill out the financial information you will be using to meet the requirement - if it's all his, then you don't mention yours.
About your documents, it looks like you've copied and pasted the list for people applying outside the UK, not for FLR(M), so your documents don't make much sense and most of them don't apply to your application. The list of documents required is on the FLR(M) form.
- there is no online application form. You print and fill out the FLR(M) by hand
- you won't have a biometrics receipt because the biometrics will be taken at the appointment
- no priority processing receipt either as you are applying in the UK
- you can just take his actual passport as he will be there at the appointment with you
- you don't need a letter of support as he will be at the appointment with you
- financial documents look fine
- for accommodation you need a letter from his landlord giving you permission to move into the property when you are ready to live together... You have to prove you have a property you can live in together.
- you don't need any photos but you can take one just in case
- you need to show evidence of regular communication while both living in the UK but not together... So I'm assuming you communicate by phone, email, visits to see each other within the UK - so things like phone bills, message/call logs, train or bus tickets, not boarding passes etc.
