1. How often do my sponsor and I meet: I wrote that we have lived together from 1999-2018 - do I need to be more specific?
You'll need to cover the period since you returned to the US as well... to prove your relationship is still subsisting (otherwise they may wonder why you haven't seen each other since September).
So, something like: 'Lived together in the US 1999- 2018, but currently living apart until I can apply for the visa"
3. For the question about children I wasn't sure if they meant from a previous relationship since it seems sort of like they are referring to children that don't live with you - otherwise should I just put our children's names and put an approximate amount that we spend on them per month?
Which question about children? Can you write the question numbers in too?
If you mean 1.27, they mean ANY children your sponsor has, including your own children together.
And secondly, yes... just approximate how much you spend on raising your children each month.
4. For the method of meeting financial requirement we are going with Category D using cash savings and my 401k
in section 3F do I check the Category D alone box or Category D combined with other income?
Category D alone, because the 401K is cash savings, not income.
