When can I apply for ILR? I believe I count 28 days, but am not sure which date to count from, date of approval or date of entry?
It’s from date of entry so you can apply 28 days before 3rd Dec 2019.

Yes, I believe so. You need mail covering the period since your last visa was granted. For most people that will be 2.5 years but as your visa is valid 5 years, I assume you’ll need to cover the whole 5 years.

Yes, I think so. One item in each name every 10 months. You need at least 3 different official sources.
3. Is the P60 for each of us a necessary piece of documentation? My husband can't find his 2019 P60 and is wondering if he needs to request one from his employer.

No, P60s are not needed. They are recommended but not mandatory.
How are you meeting the financial requirement? If you can meet it using just one of your incomes, you only need to provide evidence of that person’s employment income.
