Clean Driving Record for Spousal Visa but....?

I know people have been attaching their driving records to their application so I am doing the same.

I’m concerned about a few things that I probably shouldn’t be concerned about -

1. I got a 5mph above the speed limit ticket in the state of Iowa in 2016 but it’s not on my record
2. I also have a couple parking tickets which I know isn’t a big deal.

Should I still declare those despite having a certified and clean drivers record?

I'm not sure about the parking tickets but I'd recommend disclosing the speeding offense. Its super minor and is not gonna affect your visa (my record is way worse and I've been fine). But if they did go looking and found it and you havent disclosed it, they could accuse you of dishonesty. And in the explanation, just say its not on your record cause it was out of state. No biggie. I think most people only give their records if they have something on it as evidence of what they report

Okay! Should I just attach a separate paper to the document and have a note written/typed on there?

Are you going for the first visa? I am on ILR online application now and my previous two visas were paper applications. I have no idea what the other ones look like now.
There should be a question either way asking if you have any convictions (including driving offenses). The online form allows you to provide explanation in 500 characters. The paper one told you to explain on an additional page. So until you see that question on the application, I'm not sure what to tell you. Your explanation should be simple either way so don't sweat it.
As for the parking tickets....I'm really not sure. I feel like its not serious enough to make its own entry (unless you never bpaid them?), but I might be wrong. You may want to think about just briefly mentioning that you have had some and that they have been paid. Hopefully a more experienced person will come along to verify what you should do. But don't worry about minor driving offenses...will be fine

I feel like it’s an updated application because it’s online only and it asked me : at any time have you ever had the following?

- a criminal conviction
- a penalty for driving offense, for example disqualification for speeding or not motor insurance
-an arrest
- a caution, warning or other penalty
Etc etc

So I guess it’s the second bullet point that’s worried strangely because I have gotten a speeding ticket but I wasn’t penalized for it? I just paid it.

Yea, its worded badly. Still disclose it. If its the online system, there will be a page asking you to enter the offense, the date (you can estimate, just say you don't remember the exact date in the explanation) and then explain it. I'm assuming there was a just a fine and that it was paid? So just say that and that no further actions were taken.

Even if there was no disqualification?
Would it be better to attach a note to my driving record?
