Referee form query for dual citizenship

I was just going through all of my paperwork before I finish filing my online application for the AN citizenship process - and I noticed that there seems to be 2 different referee forms?? On the AN Application form on pages 20 to 22, there is the first and second referee forms where they put in all of their information and sign it, which is what I have. But when I was doing some research, I have found a different version of this form which is just a declaration listing the requirements and they just put their full name, signature and date:

Which form is the right one? Or do I need to use both?

Also I have just noticed that one of my referees has used blue ink. Is this acceptable or does it have to be black?

The form you linked looks like the right one. You provide their contact details in the application, and that's it from your side.

I can't answer on the ink - it would probably be OK but if it was mine I would get them to redo in black (I think we even provided a pen!)
