Help Filling Out FLRL(M) Please

I am not sure what they want in a couple of the boxes and would appreciate any help. I'll give the item number and description of the things that are not clear to me.

Section 1 - Payment Details:
1.1 Full Name - do they want my new married last name or the name that is on my passport?

1.4 Home Office Ref - where do I get this or do I even need one?

Section 2 - Applicant's Personal Details:
2.2 Full name - this is my first and middle names?

2.3 Family name - do they want my new married last name or the last name that is on my passport?

2.11 Home Office reference number - again, do I need this and if so where do I get it?

The rest of pretty much straight forward. Thank goodness I already had my fiance visa, most of the form I didn't even had to fill out and a lot of the documentation listed won't be necessary a second time either.

Look on your fiance visa...

Ok, I'm looking but not sure what I'm looking for. There is nothing on my fiance visa that says Home Office Reference number. I see a six digit number in black that says VAF No. I see a nine digit number in red in the upper right hand corner but there is nothing to tell me what that number is. I also see a series of printed numbers and letters along the bottom of the fiance visa. The first part of the series is my passport number then USA for my nationality, the second part of the series is my birthday, then the third part of the series starts with an "F" followed by a seven digit number. I have no idea what the third part of the series is for.

I think I used my new married name on the form and that worked fine, even though I hadn't changed my name on the passport yet.  (But it was awhile ago!)

Did you get a letter from the home office with your visa?  There might be a reference on that more clearly marked.

Did you get a letter from the home office with your visa?  There might be a reference on that more clearly marked.

I haven't dealt with the HO yet. My fiance visa came from Los Angeles and I received no other documents with it. If worst comes to worst I'll call and ask somebody at the HO.

Also for the family name, I have been doing it like this:    Jane Holly Jones (Smith)

Where Jane and Holly are given names, Jones is the married name with a double underline beneath it, and Smith is the maiden name in parens.  It's reversed if there's a patronymic or the maiden name is double barreled. 

To be honest, it doesn't matter much in the larger picture, but it's good to stay consistent with whatever convention you adopt.

OH!  And yes, full name on 2.2 is all of your given names.  Whereas full name on 1.1 is - in fact - your full name.   ::)  The whole form is badly in need of an overhaul.   :-\\\\

All the given names? Please don't tell me I have to include my former married name as well :P How would you write all that? I went back to my maiden name after my divorce almost 10 years ago.

Given names do not include former names, they are just the ones that were given to you...
