Is this all I need??

Okay...so, I will have:

1. Microchip registration letter, showing my dog is microchipped, the day it was inserted and the microchip #.
2.  Rabies vaccination certificates...showing my dog was vaccinated after he was microchipped.
3.  Non-EU 3rd Country Certificate
4. APHIS form 7001
5.  Rabies blood test (original)

So, I have my vet fill these out, date them 10 days before I leave and I send them to the USDA vet.  Is this all I need?  Also, do I pay the USDA vet a fee as well?

Yes, the USDA will charge, I paid $27 or $37, cant remember.  The vet will complete the 3rd country certificate, and it will include a section for the tick/worm treatment.  When are you leaving??

I'm leaving on May 24th.  I'm starting to get really nervous because I'd never forgive myself if I didn't do something correctly and my dog ended up in quarantine.

Looks like you've got everything covered.  I knew I had all my ducks in a row, but the wait to hear if my cat was being released was the longest 5 hours of my life!
