Questions on form DS-230 Part I

I thought I had read some questions here about the DS-230 Part I but now can't find it ... and I am trying to fill this out.

To make this less confusing, let's say that I am the US citizen, and my wife is the non-US citizen for whom I am filling out the form.

Questions:
1. From whom's viewpoint am I completing this form?  In question one, do I put her name or mine?
2. For Question 10 and 11, is it okay to leave both of those fields blank, or should I put something like a parent's address?

The DS-230 is your wife's form. Fill it out as if you are channeling her spirit. :)

Can you type out what Questions 10 and 11 are? Maybe I can remember what we put...

Oh, sorry.  From your detailed timeline, I was sure you had it memorized  ;) .

Questions 10 and 11:
  • 10. Permanent address in the United States where you intend to live, if known
    (street address including zip code). Include the name of a person who
    currently lives there.
  • 11. Address in the United States where you want your Permanent Resident Card
    (Green Card) mailed, if different from address in item #10 (include the name of
    a person who currently lives there).

Memorized! *laughs* I'm lucky when I remember my husband's name. ;)

For those I put the address of my parents in the US. When I consulted the lawyer, that's what he told me to do - to use the address of a relative (with their knowledge) in the US.

Are you in a situation where you could use a family member's address? What you'd do is use their address in the paperwork. Then when you arrive in the States and get your own address, you file an address change with the Powers That Be.

lol. thanks for the reminder Sara! I would have completely forgotten to change the address with them! Mom's moved house since we filed the DS-230s!
