Where do you find Form 998?

I tried calling my USDA stamp certified vet in Harrisburg, PA 4 times in one week and no one has called me back which is wonderful, but where can i find this form on my own? my own personal vet (who is part of usda) doesnt have this form and they've never had to do this type of relocation form stuff...

i saw a link for it but that was the long form version and doing searching i can only find places where you can buy it. I would imagine this form should be freely available.

oh shoot i found them. i didnt realize those were "links".. just thought it was text. there was no differential color.. it only displays if you roll over it. got it! thanks!

Yeah, I think you can set your browser setting to always underline links, but I don't think the style of this forum displays them in a different colour or anything, so unless you hover over it, you can't tell.
