Customs form help - moving back to US

Hi all, anyone know what they did or will fill out in Part IV of Declaration for Free Entry of Unaccompanied Articles form when you're shipping goods back to the US? This questions we are chewing on relate to US residents moving back to the US.

Under B), No. 8 asks if you have foreign household effects acquired abroad and used more than one year. Most of our goods actually came with us from the US, a couple of things are from here. The moving company said to just leave that blank. Is that ok? From looking around online, it seems I should check that box. Though some people say I list those goods, others say I list all goods (which obviously they weren't all acquired aboard), and some say when you check only that box you don't have to list any goods. I'm lost.

Also, on C) under Part IV, No.9, it asks if you are bringing personal effects acquired abroad. Again, most of what we have originated from the States, either when we came here or from visits (we tended to buy clothes on trips home). But of course we have a few things from the UK. Again, we were told to leave this blank. What did you guys do?
