SET M Form Question

There is a few stuff on the form that I am not sure how to fill it out.
Help pls?!

1.  The first page, when they asking for payment details.  Because I am going in for the premium service, am I'm suppose to fill the payment details out or am I'm suppose to go to the PEO office and give them my credit card?

2. In section 8 - Your Home and Finances.
Question 8.4 ask, is your partner working in the UK?
My partner is working in the UK but then I have to answer what is his pay each month after income tax and other deductions?
When they say other deductions, does it include mortgages and phone bills and etc?

3. Question 2.10 and 2.11, how often you see your partner and when did you last see your partner?  I answer for 2.10 = EVERYDAY and when did I last see partner = I see him everyday.  Did I answer it correct or ?

4.  Should I bring photo of my husband and I?

1.  I applied for FLR(M) at the Croydon office in person and they asked for the payment page filled out.  So, yes, have it completed and ready to go.

2.  Not sure on this one

3.  Yes, those answers are fine

4.  You can, but they may not want it (we had a wedding photo and the girl handed it back to us and said, "You can keep the photo".)

Yes, you should fill out the payment sheet even if you've got an appointment at the PEO.  I had my FLR(M) appointment in Croydon and didn't fill it out.  They let us do it there, but you might as well do it ahead of time.

Yup.  I think I put 'We live together' or something like that.

4.  Should I bring photo of my husband and I?  

You shouldn't need it.  You'll be there together and have documents showing you live together.
