Advice on tax (as employee)

Hello!, I would appreciate some advice about my taxes..
I was working last year as self-employee from late May until late november, earning about 1,000£ a month.

Do you guys know which forms I need to fill out to do my taxes this year? and also, when do i need to send/pay them?

I have no idea and I wouldn't like to have any problem in this matter..

For advice, I'd go for an accountant with the the Chartered (ICAEW) or Certified (ACCA) professional bodies:

You will need to do a Self Assessment tax form and also pay your National Insurance contributions.
