Tier 4 Visa Question

I am filling out my marriage visitor visa and I have to report my tier 4 visa I had last year. It says "What authority issued your visa i.e. British Embassy, High Commission, Consulate"...for some reason, I cannot figure this out (lol). On my Tier 4 visa it says place of issue: New York. Would that be it? IT doesn't say anything else.

Yes, the British Consulate in NY.

Do you know where it asks where you have been in the last ten years, if the dates need to be exact? I cannot remember all those exact dates!

Just give your best estimates for when and where you were (even if it's just month and year).
