I printed my FLR(M) form when I scheduled my in-person appointment at the beginning of April and, of course, they released a new form the next week when the prices changed. I just kept the one I had already printed for my own records and submitted the new version.
If you are worried, you can start filling it out before and then just check right before you apply to verify if there is a more current version.
