Is this the actual receipt I need to print out .. or the one on my bank statement ?!

THank you !!! Just went for NYC biometrics now finally finishing up my packet to be sent off ... I'll prob post a  finalise  list in a few checked it a Bunch of times but having someone's see if I left out anything always helps 😊
