My cat is in England! Parents flew her in cargo using Air France to Paris yesterday and we drove back to England today. My parents said nobody ever checked her paperwork and we had a quick check before taking the Eurotunnel. We used the french/english version from the USDA website.
The airline required her food to be empty at check in and said they would feed her, but it looks like they didn't. We attached a hamster water bottle above the water bowl in case the water bowl ran out and it looks like she used that to get water during the trip. My dad created a small litter box out of cardboard for her crate which worked well since her box was oversized for her to begin with (if you are in between crate sizes, or even if you're not, I suggest sizing up so your pet has more room!) She didn't use it during the flight, but it caught the water that spilled from her bowl.
Upon arriving home, she walked out the window on the second story (the roof slants right outside the window) and promptly starting sliding down the roof and I had to grab her to prevent her from sliding the whole way... Looks like we will be keeping the windows shut from now on!
