Spousal renewal questions

So my husband is pulling together information to renew his spousal visa, needs to apply next month.  We have some specific questions on how to answer a couple of things on the application and documents to send.

1) does he have to pay an nhs fee again?  we don't see anything about that on the application, i know we did pay one previously, either on his original work visa or his current spousal visa.
2) there are 2 questions which appear to ask the same thing, Current Status asks 'what type of residence permit or visa do you have'?  to which he replied spouse/partner leave to remain.  Then it asks under UK Immigration Status 'what type of visa, leave to enter or remain or other permission do you have to be in the UK?'  the answers are multiple choice and he could pick 'family' or 'work' since his visa states 'work permitted', what should he choose??
3) we have to prove my uk citizenship, i believe we did that previously by sending a copy of my passport.  my British passport expired 3 weeks ago, will that still be valid proof?  if not can i send a copy of my birth certificate?

1.) Yes. And it is now £400 per annum for the life of the visa (£1000 for FLR(M)).

2.) I can't see the questions, but if on a spouse visa, you are on a 'family' visa and NOT a work visa. Tier 2 is a work visa.

3.) Not sure on this one... passport is best.
2004-2008: Student Visa
2008-2010: Tier 1 PSW
2010-2011: Tier 4
