I am sorry to close this thread with some sad news. My wife passed away Thursday morning after a terrible battle, the last 2 weeks were unspeakably painful. Thank you all for your kindness and support. It all happened so fast, still she is at peace now.

Hello all, once again I'm looking for advice 
As my wife passed away some 6 weeks ago, I"m up against a pile of paperwork and legalese.
I won't bore you with all the details but as we lived and worked in the USA for 25 years before moving here in 2011, I'm wondering ...
As a resident alien, she had an IRA / annuity with Prudential in the states and they are asking for a 'report of Death' from the US Embassy.
Would that be treated the same? Should I send in this form with her details although she was a British citizen??

First, I am so sorry for your loss. I would think that in order to collect her SSI and IRA funds, they will need proof of her death. She collected it and you are entitled to it now. I would get everything from everywhere. The US Embassy in
London is much more knowledgeable and human than most US govt offices, from my experiences with them.
