I asked this under Visas and did not get a response,  How long before one should renew their Spouse visa? Would length of time spent in the USA effect visa renewal?

Renew before expiry from within the UK. Don’t do so less than 30 months after you entered the uk in order to avoid another extension before ilr.

There is no blanket policy on time spent out of the uk. If you spent 18 months in the us without your spouse, you’ve got problems though. You need to show you were living with your spouse for the two years before expiry, and living apart for more than short visits would be an issue.
