P60 needed? Bank tatements every page certified?

I didn’t start working until December, however my p60 shows from April 2018 to April 2019 which only includes 4 or 5 months of my pay, should I still include my p60?

Also should I have my bank statement certified on every page? The banker gave me a stack of staples statements and just certified the front page ? Should I have every months page individually certified?
