IHS surcharge for renewing spousal visa

This may have already been discussed but I'm having trouble finding specifics regarding this. Looking for some guidance. I am currently applying to renew my spousal visa (expires Nov 21). I was under the impression that if you work as a frontline NHS worker (I'm a staff nurse) that the IHS surcharge would be waived? Is this still the case? I'm trying to complete my application and it's not giving me an option to apply for an exemption or fee waiver. Thank you for any help you can provide.
