It seems the process for TOR has changed. I gathered the information the web site indicated, and I was able to go through the questions in about 15 minutes, even with triple checking everything. We were able to upload the inventory in Excel (XLSX) format, and it didn't ask for value. We didn't have to email anything, and got back approval within 3 hours!
(I'm a UK citizen, so that may have simplified things.)
