I filed streamlined for the first time at the end of last year. I used the married surname, but i don't think i ever formally changed my Social Security Card name. Will this be rejected? Should i apply for a name change now and hope it gets changed before the returns are processed?

I have used the SS card for other things like registering children for citizenship and renewing passports, i had no idea this may be an issue.

Any advice would be appreciated. Just when i thought i was on track  ???

I'm sorry I cannot offer any firm advice, just my experience. I filed mine and my wife after we were married under her married name using the offshore streamlined voluntary disclosure scheme, and it got accepted (all done by paper).

One thing you could consider is getting your name change registered with SS, and if you are worried you could always then write a covering letter referencing your disclosure and stating that your name has been updated with the SS which now should match the record with what was submitted, and stating when legally your name changed?

Again, just thinking out loud and please do not take this as formal advice!
