That’s the Consent to Verification Checks form I mentioned above. If it’s required, you should have been prompted to complete it as part of the online application.
It’s just a form you sign to say you give consent for UKVI to verify the information you have provided with the application.
Part 1 is the applicant’s consent
Part 2 is the sponsor’s/spouse’s consent, if they are jointly named on any documents you have provided
Part 3 is not relevant as it’s for a third party financial sponsor.
If you did not include it and they need it, they should contact you by email to ask for it.
