maternity certificate for Rxs/dental

Where oh where could it be? I turned in my form to the midwife a long time ago, and no sign of the certificate anywhere, no mention of it either. I need  to get a wisdom tooth pulled asap, and will have to have an Rx of antibiotics to keep infection away. I know we CAN cover it, we can cover it if we need to go private, its just I know I am supposed to have this and dont, so its sort of frustrating to know we will be paying for things when we shouldnt have to.
Moved to the UK April 2006
Married March 2007
Moved to the U.S. June 2009

Husband accepted new job in UK April 2016
Returning to UK Aug/Sept 2016!

Go see your dentist and explain the situation to him.  If it is an emergency, which it obviously is, he may be okay with doing the treatment and worrying about the paperwork afterwards.

Also, how about a call to your midwife?  Maybe she can help you out or point you in the right direction?

Thanks guys, I will do both :) Im wondering if my certificate hasnt gotten held up in red tape, or yellow or whatever color beaurcratic 'tape' tends to be ;) They kept sending me letters saying they needed more proof I am legally here as a fiancee, asked for dfs passport, blah blah, so maybe they wouldnt issure the certificate until they knew I was legally here, I'll try the overseas office as well just in case.
