Update:  HomeAgain changed the primary contact, but, I am worried about the microchip implantation report not having my name on it.  Does anyone think that this will be a problem?  Especially with all the other required PETS paperwork?

I was not the person who had my dog microchipped (by HomeAgain as well).  They should be able to send you out a form that has the information on it (microchip number, date of implant, etc..) as well as your name.  You will need this (well at least that's all I gave to prove my dog has the microchip).
Also, when I did get the form from HomeAgain, my married name was not on it, so I don't think its too much of a deal.  What really matters is your dog's name and microchip number is there and its correct.
