Thing is I can access the confirmation of my form but not my form. :-( So I just printed out a hard copy of a blank form and am filling it in just to send it along anyways.  I paid online, they took my money and am going to Fedex it all to them.  So..that's my story.  :)

(but i'm not sure if this answers your question, as i'm still half-awake and probably should not be attempting to answer questions this early ;))
