Confused on paperwork, please help..

The dogs and I are flying to the UK on 8 November.  All their stuff for PETS is done.  I am meeting with my USDA Accredited Vet on October 13th to have her fill out all the paperwork.  I then have another appointment on 7 November, with the dogs, where her colleague (also USDA accredited) will do the tick, flea, and tapeworm bits as well as the health certificate required by Virgin Airlines.  Here are my questions:

  • Can my vet fill out both the 998 Form and the APHIS form?  Or does she just do the 998 form?
  • Can I have my vet fill out the paperwork (998 Form and/or APHIS 7001) and date it October 13th or should I have her postdate it to November 5th or so?
  • Can I have the USDA vet stamp the paperwork BEFORE the flea, tick, and tapeworm treatments? I was thinking of going down on the 5th or 6th.

Any help will be much appreciated!

I don't think there's a rule on who actually fills the forms out (obviously, just who signs it!).

Yes.  I did this (and most people would - its a tight schedule!)

1. My local vet filled out all the paperwork, then the USDA vet added her signature and stamp to all of it. 

2. I think so, but I'm not sure - I did all my paperwork within ten days of flying, so I'm not sure what could have been done earlier and what had to wait.  I'm sure someone else has better/up-to-date info - julia_atlanta, maybe?

3. You sure can - I asked the USDA vet about this specifically and she said it wasn't a problem at all, that most everyone sees her before the tick and tapeworm treatment.
