notifying the NHS re: change of address

Hello!

My husband and I are moving house, and I'm not entirely sure how to inform the NHS of our new address.  Is this done via our GP?  I have looked at the NHS site and I haven't found any options for it..

We're in the middle of scheduling quite a few appointments, though, so I'd like to make sure it's done.  :)

My GP told me that when I register with a new surgery at my new address, I'll automatically be transferred over. They did print out some stuff for my new GP, though. Not my entire file, but a kind of summary thing.

This is what happened with us when we moved last year.  It was really easy, just filling out the form and handing it in to the new GP office.

I went to the new GP and they gave me the NHS form and a medical history form that they use until they get my full records from my old GP.

It's all very simple.
