Fiance visa granted but not the spousal?

Has anyone ever been able to get a fiance visa, get married and then get turned down when trying for the spousal visa?  Or maybe get turned down when applying for the ILR?  What are the chances of something like that happening.  If one has their fiance visa are they pretty much a shoe in for the spousal visa?  Or is it entirely possible to not be granted one?

15 Mar 2010 filed for ILR based on bereaved partner
02 Jul 2010 Received ILR!!!!!

most of the application for the spousal visa is identical to the fiancee visa.  If there haven't been any huge changes in your circumstances, I can't imagine you being denied.
